CENSUS REINSTATEMENT GUIDELINES AND ONLINE FORM
CENSUS REINSTATEMENTS (for students reported as Non-Attend):
- If reinstatement is requested due to an error in census reporting, fill out the Census Reinstatement Request form. The form is automatically sent to Records for processing, and the student will be reinstated into the course. No other approvals are necessary. Notification of reinstatement will be sent to the instructor and the reinstated student.
- If reinstatement is due to an extenuating circumstance, please ensure that the student has met at least one of the below criteria before initiating this form:
- attend a synchronous class, lecture, recitation, or field or laboratory activity, physically or online, where there is an opportunity for interaction between the instructor and students;
- submit an academic assignment;
- take an assessment or an exam;
- participate in an interactive tutorial, webinar, or other interactive computer assisted instruction;
- participate in a study group, group project, or an online discussion that is assigned by the school (does not include logging into an online class or tutorial without any further participation)
We greatly appreciate your efforts to ensure the student will be successful before they are reinstated. Please discuss the situation with the student and mutually agree upon a plan moving forward to meet course requirements. After that, fill out the Census Reinstatement Request form and attach to the form any documentation that supports the student’s reinstatement (email chain between instructor and student, etc.). The form is automatically sent to Records for processing and the student will be reinstated into the course. No other approvals are necessary. Notification of reinstatement will be sent to the instructor and the reinstated student.
The form is also posted on FYI (under Records) and can be found in Brightspace (under Faculty/Staff Resources, Academic Information, Records & Registration) .
The student will be reinstated in Brightspace within 24 hours of processing this request.
