SUNY Niagara Student Conduct Faculty and Staff Incident Reporting Form.

Faculty and Staff Incident Reporting Form

Dear Faculty and Staff,

Please use the Faculty & Staff Incident Reporting Form to document student conduct concerns.  You can find it on the FYI page under Academic Affairs Forms.

What to Report: Classroom disruption, disrespectful or intimidating behavior, dishonesty, and other violations of the Student Code of Conduct.  Academic integrity concerns can also be reported so they are documented. These matters are typically handled by the instructor or Academic Affairs. The Student Conduct Coordinator becomes involved when a pattern of conduct emerges.

Select “Reporting Only” if you only want to document an incident. Documentation can support disciplinary action or a written warning that outlines potential consequences if the behavior continues.

For serious incidents such as fights, significant disruptions, students in distress, or anything that cannot wait, contact Public Safety.

Thank you for your dedication to our students’ academic and personal success.
